Continuing Care


We offer an individualized Continuing Care Plan to our clients as a service established prior to being discharged from treatment if needed. By tailoring a strategy to your specific needs, your continuing care plan comes with suggested activities, including support groups, and other referrals to set you up for success.

 

Your plan will depend on your risk classification:


  • Minimal And Moderate Risk: No continuing care recommended.


  • Significant Risk: you may or may not be recommended to complete continuing care but if attending an informal or formal hearing with the Illinois Secretary of State they will request a copy of the continuing care plan, a continuing care status report and or completion of continuing care.

  • High Risk: your formal continuing care plan is required prior to your discharge from treatment. An officer from the Secretary of State will want to see tangible, substantial progress in your behavior, length of sobriety of at least 12 months along with an established support system since the completion of treatment if attending a Hearing.
