What is needed to obtain a DUI evaluation? 

A driving record for court purposes in the state you hold a driver license.  The filling is a link to Obtain a copy of your driving record online if you hold an Illinois license .(You will need the dd# at the bottom of your driver license to obtain online).

If this does not work you will need to go to the nearest DMV to purchase a copy.

Driving Abstract for Court Purposes from the DMV, past hearing documents, past evaluations, sworn reports from your arrests (these documents can be obtained by sending in the following form to the Illinois Secretary of State).

What is needed to obtain a DUI Evaluation for Court purposes?

A driving abstract (record) for court purposes in the state you hold a driver license, any tickets, police report and the Sworn Report issued at the time of the arrest.
